To enroll in First Choice Kids, you must first complete a Medicaid application. The state Medicaid office will review your application and decide whether you are eligible for the Healthy Connections Kids program. Once your child/children are approved, you will receive a letter from the state Medicaid office informing you of their assigned health plan and coverage.
If your child/children are assigned into a health plan other than First Choice Kids and you would like to change to our plan, you may call Healthy Connections Kids at 877.552.4642. A representative will help you change health plans within the first 90 days of your initial enrollment.
