Every member of First Choice Kids has a First Choice Kids ID card to show membership in the plan. If you have not received a card or if it has been lost, please call our Member Services Department at 866.299.9594, and a new card will be mailed to you.
The First Choice Kids ID card is very important. You should carry it with you at all times. You must show your First Choice Kids ID card whenever your child gets services from doctors, hospitals, pharmacies and other First Choice Kids providers. Do not allow anyone else to use your First Choice Kids ID card.
What's on Your ID Card?
- Name
- First Choice Kids ID number
- Date of birth
- Effective date — the date you became a First Choice Kids member
- Primary care provider's (PCP's) name
- PCP's phone number
- Important information for pharmacies
