Intensive Case Management
 

We have an expanded case management program at First Choice that offers a nurse case manager to our members who have many medical problems and needs. This nurse completes a health evaluation with you or your caregiver. Then, the nurse will help you and give you information about your diseases. The nurse may help you get medications and medically necessary equipment and supplies. He or she can also arrange transportation, find specialists and connect you to community resources.
