First Choice Community Center

Our community center is located in Columbia. It is a place for people to learn about First Choice’s programs and services as well as to connect to community resources and benefits. Call 1.888.676.9588 for more information.

Mission: Working together to enrich and empower our communities to better health care and quality of life.

On-site health benefit educators can provide:

• Information about the First Choice health plan
• Education on preventive health care
• Help with plan services
• Free notary services
• Orientations for new First Choice members
• Bilingual services
